Exercise Plan Form

Directions:  Use this form to plan your activities for the week.  Be sure to include whether you met your goals, any areas that were more difficult or impossible to meet, and how your plan might need to be revised for the following week.

 

1.                  Type of activity.  For example, walking, jogging, elliptical trainer, stationary bicycle, exercise video, lifted weights, etc…  Feel free to use your own shorthand for your activities.

2.                  Duration of each activity. (in minutes or ½ hour, etc.)

3.                   Additional comments.  Include whether or not the activity was convenient, enjoyable, and whether you were able to complete the activity as planned.
